- Evidence of authorization: Include evidence demonstrating that you are the authorized person to act on behalf of the owner of the exclusive rights being infringed upon.
- Contact information: Provide your contact details, including your full name, physical address, telephone number, and a valid email address. This will enable us to get in touch with you regarding your infringement claim.
- Good faith belief statement: Include a statement indicating that you have a good faith belief that the use of the copyrighted material in question is not authorized by the copyright owner, its agent, or the law.
- Accuracy and authorization statement: State that the information provided in the notice is accurate to the best of your knowledge. Additionally, affirm under penalty of perjury that you are authorized to act on behalf of the owner of the exclusive right allegedly being infringed.
- Signature: The notice must be signed by the authorized person acting on behalf of the copyright owner.
